Once you have completed and submitted the below form – and we have checked your details – we will submit this to Disclosure Scotland. You will then receive an email from Disclosure Scotland asking you to complete your application online.
We need 3 forms of I.D. to submit your application.
You don’t need a scanner, pictures are fine as long as we can clearly see text and images
At least one must be photographic e.g. Licence or Passport
At least one with your Name, Address and Date in the last 3 months e.g. Bill or Bank Statement
One more form of I.D. (can be either of the above or other e.g. your Birth Certificate)
